Open Enrollment for Physician Fellow Designation Closes midnight December 31, 2008

On December 31, 2008 APWCA will close the open enrollment pathway for physicians to become Fellows of the APWCA. After closure, physicians will still be able to join as Associate members without wound care physician certification and enjoy the all benefits the Association offers. However to become an APWCA Fellow, physicians will then be required to complete a wound care certification examination for physicians.  If you have colleagues that you feel should be members of this Association, inform them that physicians are able to join under the current policy prior to January 1, 2009.  The current policy is that physicians Board Certified in their field of medicine are Fellows of the APWCA. Inform your friends and colleagues that they can join now under that policy. Current Physician members of APWCA will continue to maintain their current designation of Associate or Fellow following the open enrollment closure.

This Association, without a doubt, has been one of the fastest growing wound care organizations in the United States over the past seven years. Nearly 2,600 professionals have joined and approximately 2,300 members today maintain an active standing with the organization.  We have been able to meet and surpass many of our initial objectives, but there are continuously new challenges that arise. Our goals and objectives are many, our work ethic to achieve relentless, our ability to accomplish is proven and our concern for membership support and optimal patient care unyielding.   Working together our potential accomplishments are infinite. Your membership is the reason this Association exists.  Each member represents a portion of the driving force that motivates our board and committees.  


Stay tuned for many new, exciting projects and important initiatives.  These activities are in their final stages of organization and we will be reporting updates to the membership during the upcoming year.  While the Physician Certification in Wound Care offered by the Council for Medical Education and Testing (CMET) has significantly changed the wound care sub-specialty, our other new initiatives will have similar impact on the wound care community.  All of these improve the state of the art and are organized in a manner that protects the Health Care Provider so that we, as professionals who care for our patients, have the ability to choose products and services, that best aid our patients and address reimbursement and similar related issues.

 

We look forward to an exciting new year and believe our membership will be energized as are our board and committee members regarding new projects.

The following highlights a few of these accomplishments and ongoing activities:


Physician Certification in Wound Care

The Association endorses a physician specific wound care certification process, administered by a separate organization, the Council for Medical Education and Testing (CMET).  The American College of Hyperbaric Medicine recognizes this as the physician certification process. Their leadership had identified that hospital administrators and physicians alike were looking for physician specific certification.  This is the first step in attempting to obtain wound care recognition as a subspecialty in medicine.  We encourage all physicians (MD, DO, DPM) to sit for the certification process so that we can demonstrate interest that exists for specialty recognition.  

 

The CMET Examination will be offered in New York City on Thursday, November 20th, 2008 from 8:30 am – 1:30 pm in conjunction with the VEITHsymposium and in Philadelphia on Sunday, April 5th, 2009 from 8:30am – 1:30pm in conjunction with the APWCA2009 National Clinical Conference – “Wound Care and the Related Sciences”.

Insurance Committee Chair Appointed: Laura Jacobs, MD, PhD, FAPWCA

This is without a doubt one of the busiest committees of this Association. During the past six to eight weeks the committee has provided comment regarding pressure ulcers, products targeted for removal from coverage and currently issues regarding pneumatic compression therapy. Much of our effort is provided through the Alliance of Wound Care Stakeholders that is organized and founded by Marcia Nusgart, R.Ph. We also work on our own as an independent Association. This provider and patient advocacy is an important daily function of this Association and totally supported by the membership………a service for our members and supported by our members!!

The following is a previous report of earlier initiatives in which the Association proudly had impact.

Competitive Bidding Delayed

At approximately 4:30 p.m. on July 7, the Senate approved, by a 69-30 vote, Medicare H.R. 6331, Medicare Improvement for Patients and Providers Act of 2008, which contains an 18-month delay and reform of competitive bidding. This bill also reverses a 10.6% reduction in Medicare physician payment allowance and also exempts physicians from supplier accreditation requirements for Durable Medical Equipment (DME). The House of Representatives had previously approved the bill with a wide, bipartisan margin.  The President’s veto of the bill was overturned by the Senate which had 69 votes in favor of the bill which was more than enough to overturn a veto.

 

APWCA is proud to have been an organization that was significantly involved in educating members of the House and Senate of some of the potential problems by implementation of the Competitive Bidding program.  While negative pressure therapy is the treatment management method that provided access for APWCA opinion, it was and still is a larger issue.  In July of 2008, CMS was to have implemented the competitive bidding program in 10 cities and 10 treatment subject areas.  In 2009, the proposal was 90 subject areas, and in 2010 the entire CMS system was scheduled to be on the competitive bidding program.  Whoever provided the cheapest service would have received the contract.  Patients who require significant care, such as those who required specialized bracing, due to muscular disease, could have fallen through the cracks without receiving the proper care that they need.  The reason for this is that bracing companies that make sophisticated devices for highly specialized needs would not have been able to compete with companies that can manufacturer a very basic brace at a lower price.  It is also interesting to note that there were not many medical organizations involved in the competitive bidding issue – APWCA was one of the most active and views this program as not only a wound care issue – but an issue impacting all recipients in the Medicare system.

 

APWCA was also very involved in opposing TrailBlazer, a major carrier of Medicare, for Texas and other areas of the country.  TrailBlazer had proposed a maximum of 3 debridements per ulcer, per lifetime, per patient.  APWCA was in the forefront of opposing and offering alternatives which provided a much better treatment regimen working with other organizations through an Alliance that is effective through the efforts of Marcia Nusgart.

 

 

APWCA is involved in issues like this on a regular basis; protecting patients by allowing providers to utilize services and supplies that can increase positive outcomes and decrease risk.

Physician Wound Care Certification Exam – Update

CMET’s certification exam moves forward.  Two examination dates and locations are now open throughout the US that are offered in conjunction with outstanding conferences including our National Clinical Conference, APWCA2009.  The certification process incorporates a for hour written response exam.  Resources for study can be found on the CMET website, www.CouncilMET.org.

 

·         Endorsed by the American Professional Wound Care Association and recognized by the American College of Hyperbaric Medicine as the premier Physician Wound Care Certification.

·         Offered by the Council for Medical Education and Testing – Not for profit status pending at time of printing

·         More than two and one half years of intensive committee work: field tested, re-field tested, and peer reviewed. The results: NOT a trivia test on interesting but not clinically relevant minutia, but rather an assessment of knowledge base appropriate for daily physician practiced wound care from an evaluation and consensus of a large randomized committee of physicians representing multiple geographic areas in the United States

Wound Care Review Course

A Great Prep for the Physician Certification Exam

 

The APWCA is developing a new course “The Essentials - A Comprehensive Review and Refresher Course on Wound Care Management and Intervention”, as a thorough review and refresher course.  This is a great two-day review for all providers and excellent for certification examination preparation.  The course is scheduled for Saturday, January 10, 8 am-6 pm and Sunday, Jan.11, 8 am-1 pm. and offers two days of intensive review, to be held the Marriott Renaissance Concourse Airport Hotel, Atlanta Airport – so weather will not be a problem with attendance!  Further information and registration available on line at http://www.apwca.org/WoundCareReview/

Call for Abstracts – APWCA2009 National Clinical Conference
Submit your oral or poster presentation abstract online.  The American Professional Wound Care Association will be collaborating

with the American College of Hyperbaric Medicine.

 

Applications for the submission of poster and oral abstracts for presentation at APWCA2009 are currently being accepted and are available on the APWCA web site. Guidelines for presentation have been revised for this year.  All abstract applications must be submitted as an e-mail attachment using the forms available on the APWCA website.  The deadline is February 2, 2009 for all Oral and Poster abstract application submissions. APWCA Reaches out with Increased International Interests
Building the Global Perspective of Wound Care

 

Dr. Kravitz along with Dr. Phillip Tutnauer, DPM, FAPWCA recently traveled to Taiwan where Dr. Kravitz lectured on wound care to several hospitals and both doctors participated in Grand Rounds.  Dr. Kravitz also recently lectured in London as APWCA collaborated with Biba Medical at a new European Wound Care Conference, iLegx. Look for more news in 2009 on global programs.

 

Ghana, Africa:

An update on the Buruli Ulcer, a devastating ulcer disease will be presented during the APWCA 2009 National Conference.  We want to acknowledge Dr. Darlene McCord and the McCord Foundation for making a commitment to Dr. Ampadu and his work with the Ghana Health Service to battle against the Buruli Ulcer that commonly manifests itself in children in Africa.

The American Professional Wound Care Association (APWCA) is a collaborating organization with VEITHsymposium.  Wednesday, November 19, will be the 3rd annual WOUNDS at VEITHsymposium.  Dieter Mayer, MD, Head of Wound Care at Zurich University Hospital (Zurich, Switzerland) serves as director, along with a group of top-flight faculty. 

WOUNDS is a distinct one-day comprehensive, program for the vascular specialist, podiatrist and other members of the wound care team interested in all aspects of wound care.  Rapid fire presentations, followed by spirited discussions, including state-of-the art technology, techniques, and controversies are the keystone of this one-day meeting.  WOUNDS at VEITHsymposium™ is included in the full Symposium registration fee.  A per diem rate of $295 is available for those wishing to register solely for the Wednesday, November 19, 2008 sessions of WOUNDS.  A special rate of $145 is available for wound care nurses.  This activity has been approved for AMA PRA Category 1 Credit(s)™.  8 Hours of Continuing Education Contact Hours are approved by Temple University School of Podiatric Medicine, an approved sponsor of the Council on Podiatric Medical Education. Breaking News:   The American Professional Wound Care Association will be collaborating with The American College of Hyperbaric Medicine (ACHM) at the APWCA2009 National Clinical Conference. We extend a warm welcome to all members of the ACHM and are pleased to have their participation with our scientific program.  This brings APWCA2009 to a new level that has not been available in wound care educational conferences prior to this event!  Collaborating societies with the April meeting will be: ilegx (BIBA Medical – our European collaborating society) and Temple University School of Podiatric Medicine.   We are please again this year that the conference will be co-sponsored by Temple University School of Medicine, Albert J. This white paper provides basic recommendations determined by an APWCA Committee and is based upon committee and panel experience on establishing and maintaining a successful wound center. Its purpose is to serve as a guideline around which further thought and discussion should be held and is not designed to represent a definitive treatise on the subject for any particular center. We want to acknowledge the core committee and an additional 50 APWCA members who reviewed the monograph and whose comments were incorporated into this document.

WUWHS: Conference Recap

 

A big congratulations to Gary Sibbald MD, M.Ed. FAPWCA, chair of the 3rd congress of the World Union of Wound Healing Societies, which was held during the first week in June in Toronto Canada.  The meeting was a tremendous success and the APWCA directed the Diabetic Foot track and was well attended through the entire meeting.  APWCA estimates approximately 5000 total attendees and possibly more.  The meeting served as a great forum for international discussion among international registrants.  APWCA has made direct contacts for potential future events in Israel, Australia, Taiwan, Japan, and more!  Again, congrats to Dr. Sibbald and his entire committee… APWCA is proud to be a hosting society and we look forward to working closely with Dr. Sibbald over the next four years as he is President of the WUWHS.

 

The 4th congress will be held in Japan in 2012.

Pressure Ulcer Updates

 

On June 13th the APWCA Insurance Committee submitted comment to the Honorable Kerry Weems, the Acting Administrator for the Centers for Medicare and Medicaid Services concerning the Proposed Rule published in the April 23, 2008, Federal Register titled, "Medicare Program; Proposed Changes to the Hospital Inpatient Prospective Payment Systems and Fiscal Year 2009 Rates”.  The APWCA comments concerned the code descriptions for hospital acquired conditions for pressure ulcers and the proposed quality measures. 

 

The Hospital Acquired Conditions (HAC) provisions in Medicare regulations required hospitals to begin reporting on their Medicare claims on October 1, 2007, whether certain specified diagnoses were present when the patient was admitted.   In the current proposed rule, this includes pressure ulcers. CMS proposed to create new codes to better identify pressure ulcers.   The APWCA expressed the opinion that the new codes do not reflect pressure ulcers accurately. The letter explained the need to establish codes for deep tissue injury and unstageable ulcers.

 

The Committee also expressed concern that Stage III and Stage IV pressure ulcers should be a measure of quality care and be considered for non-payment should they occur during a hospitalization. Not all hospital acquired pressure ulcers are preventable. 

 

The above also reflects sentiment that was expressed during June’s meeting with the Alliance of Wound Care Stakeholders.  APWCA is a member of the Alliance, which is directed by APWCA member, Marcia  Nusgart who serves as the Executive Director.  The Alliance serves as an excellent forum where associations that have an interest in wound care can review and discuss issues pertinent to this field of medicine.

Previous Insurance Update

 

The APWCA Insurance Committee continues to be actively addressing such issues as: competitive bidding and coding for Anti-microbial dressings.   Regarding the latter, we have requested consideration for special codes for any microbial dressings which would then allow for appropriate coverage as these are more expensive, but often very beneficial to patients with infection.   Regarding competitive bidding; as a reminder the Association has concern that only cost for service will be considered for which vendors are able to provide service to Medicare patients.  

 

Currently negative pressure therapy is THE wound care therapy that is affected for this coming year, but in 2009 there will be a significant number of services added to the competitive bidding process.  In 2010 it is likely that the entire Medicare system will be based on the competitive bidding model.  Negative pressure is a complex form of therapy used on very compromised patients and must be applied in a consistent manner with materials that are effective.  It is the APWCA’s opinion that VAC should continue to be available to physicians who prefer to use this for their patients.  The competitive bidding model should be applied in a slower manner to ensure that the physicians have choice as to which type of NPWT they choose to use for their patients until further research on other forms of NPWT is available.  The following is a portion of a release from KCI (http://www.kci1.com):

SAN ANTONIO--(BUSINESS WIRE)--May 15, 2008--Kinetic Concepts, Inc. (NYSE: KCI), a global medical technology company with leadership positions in advanced wound care and therapeutic support systems, today announced that it will not be a contracted negative pressure wound therapy (NPWT) supplier in phase one of the competitive bidding program administered by the Centers for Medicare and Medicaid Services (CMS). The program covers Medicare home placements in 10 designated metropolitan areas and is scheduled to take effect in July 2008.

"We continue to believe that NPWT has been improperly included in competitive bidding due to a demonstrable lack of comparable or effective alternatives to KCI's V.A.C.(R) Therapy system," said Catherine M. Burzik, President and Chief Executive Officer of KCI. "We believe that the CMS bidding and contracting process does not ensure access to effective NPWT in the home, and Medicare beneficiaries are likely to be harmed as a result."

 

 

 

APWCA2008 formally recognized as a non-profit organization

 

The American Professional Wound Care Association (APWCA) is a hybrid non-profit organization unlike any other organization available and founded in 2001. Its non-profit status is a 501 (C) (6) with an effective date of April 2001.

 

You can be proud of being a member or participating thorough Industrial Support of the American Professional Would Care Association.  The APWCA is a non-profit 501 (C) (6). 

 

By way of background, the APWCA never had any shareholders and no individuals have ever had financial interest in any aspect of the association.  That said, application for non-profit status was made in the summer of 2007.  After a full review by our legal council, audit by our accounting firm and review of records by the IRS the non-profit status was granted retroactive to an effective date of April, 2001.  This was the first month that this association functioned and the first month any members were accepted.  The scrutiny of the review of our documentation that was performed in 2007 reflects the integrity, honesty, and professionalism that this association meets.

 

Additionally, our Executive Director and founder has been a noted speaker in his podiatric profession for the past 20 years, Dr. Kravitz voluntarily donates any honorariums or speaking money that he obtains to the APWCA. In fact whenever possible, he requests that the money for same be drafted as a grant to the association rather then be provided to himself.

American Wound Care Foundation Update

American Wound Care Foundation is up and running!  This has been a long term project that has finally materialized.  The foundation is a Philanthropic not profit charitable organization.  Its primary goals and objectives are to provide medical supplies and services to those with limited income who are not able to receive such either because of insurance denial or lack of insurance coverage.  The Foundation will also provide education to patients and caregivers.  Those who would like more information can contact the Foundation by clicking on the button on the APWCA website homepage or visiting http://www.AmWCF.org/

CME Audit Update

 

Fellow, Diplomate, and/or Associate Members are to obtain 21 hours of continuing medical education credit in wound care every three years to maintain their designation of Fellow, Diplomate or Associate.  APWCA conducts yearly random audits of its membership.  When audited, it is important that the member maintain proper documentation to fulfill this requirement.  This can be done through attending APWCA educational events or any one of a number of educational meetings approved by the APWCA Scientific Committee.

 

Point to Note:  APWCA will automatically track the CME for each member at all of our national meetings, including APWCA2008 this past March and the upcoming APWCA2009, to be held April 2 – 5, 2009.  Therefore should an audit occur, any member who attends an APWCA national meeting will automatically default to the conference database.  If a member attended 21 hours or more of education the CME requirement would then therefore be met.  This is a major advantage to members attending our annual Spring educational events.

Name Change Announced for Honey-Based Product Line


DermaSciences, Princeton, NJ, and its partner, Comvita New Zealand, Ltd, have changed the brand name of their honey-based product line to MEDIHONEY. Both companies previously intended to market the products under the API-MED brand. The MEDIHONEY brand was acquired by Comvita through its recent purchase of Medihoney-Pty Ltd, a former subsidiary or the Australian-based Capilano group. MEDIHONEY is a line of active Leptospermum honey-based wound and burn dressings used in several countries. The first of these dressings, a calcium alginate dressing with active Manuka (Leptospermum) honey was cleared for use in the management of wounds and burns by the Food and Drug Administration in July 2007.
